Our members now have the option of reporting a Workers' Compensation claim in a more timely manner by completing the attached Workers' Compensation-First Report of Injury or Illness form. This form is in PDF format meaning you can enter the information directly from your computer. Once you have completed the form, click on "Submit", and the form will be automatically emailed to personnel in our claims department. Please print a copy for your records as well. You will be unable to save the completed form on your computer.Be sure to complete this form accurately and promptly to ensure prompt assignment of the claim.

Emergency claims that result in death or serious injury should be reported immediately by calling Darlene Frick at 803-309-9761.

The employer should direct all medical treatment to a Doctors Care. The location of these facilities are included in your new busines and/or renewal package. In the case of an emergency, the injured worker can be referred to the local emergency room.

Please note, a drug and alcohol test is required to be obtained on the date of the accident.
